Terapie zachowania płodności-nadzieja dla pacjentów przed terapią przeciwnowotworową
W celu umożliwienia młodym pacjentom z chorobą nowotworową posiadanie potomstwa w przyszłości powstały metody, które chronią ich komórki rozrodcze.

Radioterapia i chemioterapia, mimo iż bardzo skuteczne w leczeniu nowotworów, mogą prowadzić do niepłodności. W celu umożliwienia młodym pacjentom z chorobą nowotworową posiadanie potomstwa w przyszłości powstały metody, które chronią ich komórki rozrodcze.

Nierzadko celem wyleczenia z nowotworu, bez ryzyka nawrotu, stosuje się bardzo agresywne leczenie. Chemioterapia, działająca ogólnoustrojowo uszkadza bardzo wrażliwe jajniki i jądra, często prowadząc do bezpłodności. Radioterapia z kolei, jeśli stosowana na okolice narządów rozrodczych, uszkadza materiał genetyczny gamet. W tym przypadku, nawet jeśli poczęcie jest możliwe, istnieje duże ryzyko urodzenia dziecka z poważnymi wadami genetycznymi.

Kriokonserwacja oocytów, czyli komórek jajowych, nadaje się dla pacjentek młodych, które nie mają jeszcze stałego partnera. Jeśli pacjentka w momencie rozpoznania choroby nowotworowej ma partnera lub męża, z którym chciałaby mieć dzieci, możliwe jest także zamrożenie zarodków. Niestety, obie te metody, kriokonserwacja zarówno zarodków jak i oocytów, mają istotną wadę. Celem pobrania odpowiednich komórek jajowych, wymagane jest opóźnienie terapii nowotworu, co w niektórych sytuacjach może stwarzać zagrożenie dla pacjentki.
Alternatywą jest kriokonserwacja tkanki jajnika, która zostaje pobrana bez stymulacji dojrzewania oocytów oraz konieczności opóźniania leczenia raka. Po zakończonej terapii przeciwnowotworowej, tkanka jajnika zostaje ponownie wszczepiona do ciała kobiety. Dzięki temu możliwe jest pojęcie normalnej pracy tej tkankiW przypadku mężczyzn zmuszonych do podjęcia intensywnego leczenia przeciwnowotworowego, rozwiązaniem jest zamrożenie nasienia. Może ono być latami przechowywane w odpowiednych warunkach, dzięki którym nie straci na swoim potencjale .
Z rozwojem medycyny leczenie onkologiczne jest coraz mniej inwazyjne. Niestety, jeszcze długa droga zanim nie będzie ono miało wpływu na płodność pacjentów. Dlatego skuteczną alternatywą jest zabezpieczenie płodności poprzez kriokonserwację gamet lub zarodków.
